We have done this for a number of years and are paying the price.  We've
recently required everyone to meet a basic set of rules, including
naming conventions, while we fight for a budget for an EDMS.  Might as
well whistle in the wind.  Some follow the rules, some don't, and I
spend too much time having to monitor the bloody thing.  I have been
using the example of putting all your paper documents/records,
regardless of content, into a big cardboard box in the middle of the
room, and trying to find one particular document in a hurry.  Some folks
would put documents into labeled folders first, some would stack the
docs and folders neatly in the box, others would not use folders at all,
may even not use fasteners for multi-pages, or would just throw the docs
in the box with no labels.
