Does anyone know of any links to articles about cases where an 
organizations inability to produce consistent/accurate documentation 
(processes, procedures, customer user guides, manuals, etc.) led to 
them being sued or being shut down or fined by a regulator.

I'm trying to help make the case for why content should be managed as 
an asset and need some examples of organizations that have failed to do 
so.
