Once upon a time I worked for the Environmental Protection Agency, which is also tasked with creating Administrative Records, and I assembled these for the Superfund Division of Region 7.  At that point in time there were no resources available to assist in this process but over the years I got pretty good at it.  For the EPA we included any major decisions and why they were chosen.  The Record began with the discovery of the site, the testing which discerned the contamination and the extent, and then the cleanup options, the final decision and why it was chosen.
