Emma -

You might wish to avail yourself of a couple of possible resources. The
former Information Management Services Unit (IMS) of the Ontario
Government's Shared Services Bureau has moved over to the Archives of
Ontario. If you contact Carolyn Heald in the AO policy unit
([log in to unmask]), she will be able to direct you to
any of the Records Analysts who may have done work for the Ministry of
Health. These folks might be able to give you some direction.

Alternatively, you could contact the Chair of the Archives Association
of Ontario's Health Archives Interest Group (HAIG), who is:

Joseph Travers
HAIG Chair
College of Physicians & Surgeons of Ontario Archives
80 College St.
Toronto ON, M5G 2E2

Cheers!

Subject: Retention of ADP forms

Hi all,

This question is really aimed at those of you who are working in the
Canadian Health Care Sector.

My Director of Health Data Resources would like to know if there is a
retention period for ADP Forms (Assistive Devices Program). Both she
and I suspect that they are financial records and probably don't have to
be kept for more than 7 years, if that, but I would like the opinion of
those of you in the Health Care record keeping world.

Looking at the forms, they do not seem to contain a huge amount of
Health data and considering that the point of the program is to
financially assist Ontario residents with long term disabilities to
obtain assistive devices I am guessing that they are classed as
financial records.

Thank you in advance
