Indiana University, where I teach, used to be one of the leaders in this field: permitting hackers to get names, addresses, phone numbers, and social security numbers. One of the things they did, besides having poor computer security, was to track every single person on campus: faculty, students, and employees, using social security number. My class rosters were put in my mail box with each student's social security number carefully placed next to their names. Only this year has the university begun to use a different unique identifier for each person. To do this, they had to implement PeopleSoft (but that is another story for another day).

My understanding is that this is a very large problem that is only being addressed now within universities.
