I don't have one myself but, New York State has a pretty good one that I
was thinking of using as a model. It's available if you Google
"electronic signature best practices."

Subject: Signatures on Electronic Records

Hi List,

 

Does anyone have a policy on accepting electronic or voice signatures as
official on electronic records?  This could include entering a PIN or
recording a person's voice signature as official.  If no policy, do you
have a Best Practice?
