If you are looking for samples, one place to start would be agencies that
have responsibility for supporting records management programs within their
state.

Example include:
- the Library of Virginia  (

- California State Records Management (
http://www.dgs.ca.gov/osp/Programs/CalRIM.aspx)
- Texas State Library (
https://www.tsl.texas.gov/slrm/recordspubs/forms/state.html)

I suspect you can also do a Google search and find others.  But these are a
good place to start.
