Yes!! on the Board Agendas/Minutes.
Thank for the criteria's stated.
I do already have a form. However I was seeking others that might have been in use and active.
FYI to all.. As we know Departments cannot advise RM what they want or need to Scan. We are the advisers through our  Records Retention Schedules. I am working seamlessly with the IT Director who is very supportive in all aspect. He feels repeat FEELS that the best approach is to have a form and for each Dept to state what they would like Scanned. After gathering all of their  Data. I will make a decision based on  my  Retention Schedule what should be Scanned.
Any other ideas , suggestion  or  FORM that is been used available from anyone?

While I do not have a form handy to share with you, if you are
considering making one from scratch, these are some elements that will

I have been given the task to create a Document Imaging Inventory Form
which would ask the  Question:
1. What document would you like to scan in your Department.
I have already implemented  scanning, of HR Records/External
Transcript/HR Benefits/Financial Aid requested Documents/Board
Agenda/Minutes and soon Contracts.
If you do have s tailored Form available and don't mind sharing, I would
be grateful.
Please  send it in a format where I will be able to edit.
