Our box-level database table includes these fields:

(Storage Vendor) Box #
(Our) Box #
(Our) Company
Department
Record Series Title
Description/Range
Date Box Created
Year (of contents)
Retention Period
Disposal (Eligibility) Date (Used to develop the destruction review
list.)
Destruction Date (Actual)
Destruction Authorization Form Number (referencing the individual
authorization form, for easy look-up) 

Please note that we have a separate table in the database that includes
a file level entry for each file in each box. We use this table for
searching. We use the box-level table for managing the box.
