COMPLIANCE REMINDER – REQUIRED SEPARATED or RETIRED EMPLOYEE DOCUMENTS
As a supervisor at Qwest, you play a key role in ensuring the company’s records are complete and accurate, including proper administrative documentation when an employee leaves the company. According to Human Resources and Risk Management, you are the supervisor of record for the following recently separated employee(s):
Name and (CUID): John Hogan (jphogan/0085516), Separation Date:04/02/2009.
Human Resources and Corporate Compliance policies require you to fill out specific documentation and take appropriate actions as part of the separation process. Supporting information can be found on HRxpress under “Supervisor Tools”, in the “Separating Employees” section.
*** ACTION REQUIRED ***
Fill out a “Separation Checklist” or “Retirement Checklist” (formerly known as an “Exit Checklist”) for the employee leaving the company. The “Separation Checklist” or “Retirement Checklist” form can be found on HRxpress on the “Forms” tab, under the General section, “Separation / Retirement” section.
Send the completed checklist and the former employee’s personnel records to the Qwest Corporate Records Center for retention and archiving.

Ensure you label the files/folders in the envelope with the separated or retired employees’ cuids and employee numbers.

If it is necessary to retain the employee's personnel file and exit checklist due to a legal or Human Resources/ Labor Relations related matter:
* Copy the exit checklist and other documents from the employment record for your use in the field, and send the original exit checklist and employment record items to the Records Center.
* If the Law Department or HR requires the actual originals be kept temporarily the field, send an e-mail to [log in to unmask]<mailto:[log in to unmask]> explaining what originals are being held in the field until the situation is resolved. Upon resolution, send the originals to the Records Center.
You will continue to receive periodic reminder e-mails until the Separation Checklist or Retirement Checklist and any additional required records are received by the Corporate Records Center.
