To your question on how does one start a program from scratch?  

A couple of things - if you don't already have support from the top of the organization, your first step must be to find (or create through marketing and education) an executive level "champion" who understands the importance of a RIM program in managing information assets, arguably the most important assets of any organization next to people.  

Also, remember that increasingly larger volumes of records will be electronic and make sure your program educates to that realm and addresses the related issues from the beginning.

Good luck (if you haven't read Langemo or attended a seminar he conducts on this topic, that would be very useful).
