I would recommend you look at the ARMA bookstore, each of us has a favorite author, but many of the publications in ARMA cover all of the nuts and bolts of RM. Unfortunately we a still building our library. 

1. Actually starting an RM program from scratch - 
	Managing Records & information Programs (Principles, techniques & tools) by Bruce W. Dearstyne, Ph.D.
	Understanding Data and Information Systems for Recordkeeping by Philip C. Bantin
2. Placement of the records management program within the organization (meaning, do you put it as part of IT, Legal, etc) This is a business decision though I am sure depending on author there are plenty of pros cons though I cannot identify any specific publication I would say that RIM should not be any place other than IT or Legal.
3. The intersection of records management and historical archives - (loaded gun not touching this point)
