Some points to tweak your definition:

A record is a type or subset of "document" (documentation is a process). One record is data; several related documents that show direction, meaning, current situation, decision to be made--these  constitute information. A record is created or received by [not "at"]. It may be essential (vital) or of less value. It is kept for a specific period of time (as shown in the retention scchedules).
