One earlier case I recall where notification was delayed by months involved
Bank of America, but the circumstances were quite a bit different-   there
was a question as to whether tapes had actually gone missing, or if they
were destroyed in the air cargo baggage handling mechanism and the EffBeeEye
gave them 'special dispensation', because until they were done investigating
the case, they didn't want to divulge the description of the contents of the
tapes.

It never ceases to amaze me the depth of information that gets placed on
removable media or devices and not encrypted.  One would have to wonder if
whoever was using this data really needed to have all of that financial data
tied to each subscriber, or if they could have gotten by with a subset of
the information.

Over the past 6 months, I've been asked by various institutions on multiple
occasions to provide an SSN as part of my identifying information (doctors
offices, schools, others) and have refused to provide it.  This week I told
the receptionist and office manager at the Doctors office (a doctor I've
been seeing for 12 years, mind you) that I wouldn't provide an SSN on the
form they asked me to fill out that I was reluctant to fill out in the first
place.  I asked why they needed the form, and they said "we're entering data
in a new record keeping system, and these are the fields it needs" and I
asked why they couldn't gather it from the existing charts and they said
":that's not convenient"  so I asked what would happen to the form after the
data was entered and they said it would be shredded... then she said "oh,
you forgot to provide your SSN in these two places" and I told her that I
hadn't forgotten, they just weren't entitled to that information and that in
California, a SSN is not to be used as an identifier any longer, with the
exception of cases involving taxes or interest payments, that they should be
using my medical ID number only.   After about 10 minutes of arguing, they
finally realized they weren't going to get it, so they backed off... but
it's amazing how many people continue to provide this info when requested.
