I agree with Fred simpler is better. Think of your policy as the legislation
that enables your program. It says this is what you will do. Your procedures
are analogous to regulations. They say this is how you will do it.

I wouldn't necessarily put them all in one single document, but rather into
would think of them sections/tabs in a manual.

Your policy should be issued by the highest group within  your organization
e.g. the Board of Directors. It should be difficult to change policy since
it affects the entire organization. If you include procedures as part of
your policy then that same organization will have to ratify procedural
changes resulting from laws/regulations/business needs. Procedures should be
changed when expedient

I agree with your outline. I would suggest that  you try to keep it to less
than 2 pages
