Now that I have a few minutes, I will add my agreement on top of the list below.

Policy should be a separate document.  There was a good list of components that I saw go by in the discussion.  It reminds me of a (records management) policy structure I recently saw a a very large, well known, financial institution, that was a client I was working with.

The procedures, standards and guidelines can be referenced, and the policy should be approved by an authority at an executive level who has the power (or delegates it) to enforce the requirements.
