I report HIPAA breaches on a routine basis.  We have to report ALL breaches
or suspected breaches, even if the person who received the information
inadvertently, has been trained.

This article doesn't limit itself to discussing "how to analyze" but also
gives resources to determine "what comprises" a breach of HIPAA covered
information, based on HHS Sept 2013 final regulations.

One of the critical things the article mentions is assessing the personnel
working with the information and ensuring they are properly trained:

"Consider whether the unauthorized person is trained in HIPAA compliance,
has obligations to protect the privacy and security of the information, has
a track record of protecting similar information, and can be obligated to
return it.".
