I don't, but if I did, I doubt that this would have made any difference. I use Iron Mountain, and similar incidents have happened with them - and with other vendors.  The best that any of us can do is to follow up, keep our eyes open and double check them from time to time.  At one time, I insisted that either myself or one of my staffers (back in the days when I had a staff) witness all of the shredding, but now that I'm a one-person operation (well, 1 1/3 persons) there is simply no time available.
