I have designed a form for 4-5 signatures along with a list of files for
destruction.  The form has to be signed off by main managers, directors,
etc, including the legal department for approval of destruction.
Electronic would be the same in email form, however, you would have to
make sure to follow up in case someone delays the process.

The problem I see with your proposal is that it would provide you with
no
feedback if some manager disapproves of the destruction.  You would only
have a void of information, not knowing if the right managers received
the
form, whether or not they considered it, and, if they decided not to
approve
the destruction, what the reasons were.  You wouldn't know what the
manager
wanted done with the records, nor how much longer they should be
retained.
I'd suggest the form come back to you, regardless of whether they
approve or
disapprove.
