AHIMA (American Health Information Management Association) has the
following standard:

"When correcting an error on paper, the provider should draw a line
through the original entry in such a way that the original entry remains
legible. The provider should not alter the original record in any way by
trying to erase or remove the incorrect information. The provider should
print the word "error" at the top of the entry, and sign, date, and time
it. The reason for the change should be indicated as well as the
discipline of the person making the correction."

They also have standards for correcting computerized records on the
Practice Briefs portion of their website www.ahima.org.

Does anyone in the medical/dental records field know of any standards
for changing entries in a patient's records?  Do standards exist, or is
there simply a common practice?  Is this type of thing regulated at all?
