All this talk about poor recordkeeping causing/not causing disasters brought to mind an old Arthur C. Clarke story, The Nine Billion Names of God, about a Buddist sect seeking to compile all the true names of God. The kicker was their belief that once the list was completed, mankind would have achieved it purpose and the world would cease to exist.
A fictional case of good recordkeeping causing a disaster!

Unless you work in the Aviation industry - it happens. The Australian Transport Safety Bureau http://www.atsb.gov.au/ (the Australian equivalent of the National Transportation Safety Board) often has reports where poor recordkeeping has (albeit indirectly) contributed to a incident or accident.
Every industry has its potential disasters waiting to happen if recordkeeping is not up to scratch.
