We see these sorts of things too often.  But we never seem to discuss  
how we would or could stop this in organizations?  What role could a  
records manager play here?

Say for the moment;  you are the records manager of this organization  
and the government authorities come to you and ask "How could this  
happen?"  "What security procedures were breached?"  and "As this  
firms records manager, you are charged to classify and protect vital  
records.  How could files this important simply walk out the door  
with an officer?"

Canada specifically calls for protection of this type of information  
in their privacy of private citizens information laws. So someone  
here was negligent.
