you can already do this yourself. Obtain a copy of your medical records from
your health care provider. Scan the documents and download them to a
thumbdrive. Carrry the thumbdrive with you when you travel. Every time you
visit your PCP, request of copy of the pages that have been added to your
record.

In case you are incapacitated carry a card in your wallet that states what
medicines you are taking, any current health issues (diabetes, pacemaker,
etc). the card should state that your thumb drive carries medical
information as well as the name and numbers for the individual who can make
medical decisions for you if you are unconscious.
