We received a request from that very same Texas Department of Health, asking
us to complete a survey.... a very detailed medical survey.  It turned out
that the conduct of the survey was outsource to a firm that could not fully
explain how any of the information would be kept private.  Well.... we opted
to NOT RESPOND!   At present that appears to be an even better decision than
we thought at the time!
