Dear Colleagues, 
I am conducting an informal survey of medical device, biotech, and
pharmaceutical companies.  I am interested in knowing how many companies
have a Records Management/Knowledge Management type program.  I intend to
use this information to write an article which will be submitted for
publication in a  journal.  I have included a link to the online survey
below.  You may answer as many or as few questions as you please as all the
fields are optional.

I ask that you indicate what company you represent (this is so I can
recognize any duplicate responses that might skew my data).  Your company's
name will not be disclosed in the published report, though descriptive
information may be used (e.g." American company with 500-1000 employees" etc.)
Thank you in advance for your participation.  I'm sure the results will be
interesting!
