Speaking only for myself on this subject, I wouldn't use a vendor that is not NAID AAA certified.  Certification is a necessary criteria for a vendor to even be in the running for my organization's business.  I have it written into our contract with shredding vendors that they are required to keep their certification current; a lapse in certification for any reason is considered a material breach of contract by the vendor, allowing us to terminate the contract.

Regarding whether or not you shred on or offsite, and, if onsite, whether you witness the entire process, I believe that should be determined at the policy level for your organization.  I'm in a regulated industry, not government, but we use the offsite option, so we never see the process.  We feel locked bins and trust in our vendors are enough protection for us.
