I'm not certain there are any good rules for this. It is particularly
challenging in the case you cite because the form's reverse is not actually
blank. My sense, in this case, would be to scan and retain both sides of
the form, whether or not it is filled out. That way, no one can claim that
you omitted necessary information.

In a past life, I managed an imaging operations team. We always performed
duplex scanning of documents. However, the system had logic built in that
would measure the "size" (number of bytes) of an image and discard images
below a certain byte count. We considered these to be "blank". The system
was sensitive enough that even a stray pen mark would cause the image to be
retained. We chose this method because it was repeatable and easy to test.
The vast majority of forms were one-sided, so it reduced our storage
requirements considerably with no real extra effort in prep or QA -- and no
judgement calls.
