Folks, it kind of comes down to volume - and whether you really must thoroughly destroy the media.  At the US National Archives we have a few facilities (including ours in Fort Worth, Texas) that can destroy media in a variety of ways.  Our FRC (Federal Records Center) here in Fort Worth has a media shredder that enables us to thoroughly destroy CDs, diskettes, DVDs, and mag tapes to a shred size of 3/32". Our FRCs in Seattle and Philadelphia also have the same equipment - and capabilities.
 
Additionally we have an NSA approved degausser and a hard drive crusher.  This equipment - and service - is also available at the Philadelphia (Mid-Atlantic) FRC.
 
Of course these services are restricted to U.S. Federal agencies.  Over the years we found that increasingly we were encountering boxes for disposal that contained mixed media - or we had agencies that had a need to dispose of media and / or hard drives.  They didn't want to risk having the contents show up somewhere when they thought they had destroyed the media.  You hear of all kinds of "home grown" approaches to destroying media - such as skeet shooting with shotguns, putting them on train tracks, running them over with 18 wheel trucks, etc.  Actually you can view some of these on YouTube - and they are entertaining.
