David - I believe your observations are very interesting but I'm not sure I would agree with your conclusion " bureaucracy [is the] direct results of having an "external master."  

Peter Drucker held that bureaucracy appears as a result of focusing on rules instead of results. While many rules are driven by external constraints such as regulatory requirements, the focus of the organization is exclusively internal. Failing to focus the organization and its employees creates an environment of fear and doubt that leaves employees in a mindset of avoiding mistakes, rather than innovating to find new successes that align with the vision of the organization. Or, to put it another way, poor leadership provides the fertilizer for growing a healthy bureaucracy. 

People hate to be managed, but they want to be led. This is especially true of knowledge workers who are not stimulated or motivated in the same way as workers who complete repetitive tasks. A friend of mine introduced me to an especially good video on this subject. Here is the link for those who may be unfamiliar with RSA or who are interested in this subject.
