Good morning. Sorry for the delay in response but I missed this on my review of the list Friday. One of my many hats has been as a project manager for a software product that scans application forms in 32 different states for the federal free and reduced meal lunch program (including printing the application forms for the districts in multiple languages). Based on that experience I strongly recommend that you contract with a certified translation service to provide for English to Spanish form translation and design.

There are too many variables to take into consideration when formally translating business documents which are not addressed by Google and other similar products and if you are translating a federal form I would use only a certified translation. The costs are actually pretty reasonable and some translation services will actually help you with the form design itself (which can be tricky since Spanish can take up to 15% more verbiage space then English).
