I too think a spreadsheet (like Excel) is sufficient in most instances. Say
if you're a multinational firm involved in litigation on a too regular
basis, then I'd recommend something more robust. Like others have mentioned,
either an off the shelf application, or modifying or tapping into a document
mgmt. system that's already in place. Most of them, especially those
designed for legal records include a litigation hold module.

It's really pretty simple -- it's just a matter of knowing what you have,
where it is, ensuring that it will not be altered, and sharing that metadata
with the appropriate people.
