IIRC HIPAA allows for the patient to gain a copy of their medical
record. Patients need to be very proactive. If you change physicians
always ask for a copy to sent to your new provider.

this is not unlike the banking industry which employs folks to monitor
the death notices in the local newspapers to spot clients who have
passed on and they can then lock down the account.
