Thanks to those who have contributed to this thread.  I once
found/learned, etc. of a circumstance that came close to creating one of
those front-page stories regarding failure to protect personal
identifying information...... when a contractor that had not really kept
up with trends relating to identity theft, etc. exposed customer records
to disclosure by allowing them to be picked up by a recycling company
that did NOT ensure that they were first shredded.  

That recycling company's focus was on recycling newspaper records.....
and not on protecting information from disclosure.  Fortunately, the
situation did not become a problem issue..... and the contractor did
take steps to change its policies and now ensures that all of the source
documents are shredded before they're hauled off elsewhere.
