than what they were told to do, ie: "File this in the Jones case file."
The didn't go through the file and ask about duplication, copies vs.
originals etc.>

Very true, most clerical staff did not practice good records management.
However, they were the folks that did all the typing, filing,
organizing, etc. no matter how inefficient.  And there was always the
"Agnes" of the office.  She who held the secret of the files (her own
formula of course - none of this best practice nonsense).  To access any
of the materials in her lair, one jumped through hoops of Agnes's
making.  And when she left - chaos ensued.  

The point Fred made was if one's position was at a certain level or
above in the organizational hierarchy, one did not do those "clerical"
things.  Now, one does (or should do).
