9 of the physicians cited in this short list (12 of the 70) were either
cited, lost their licenses, or are required to attend specific training
because of their lack of proper medical record keeping.  4 were specifically
related to failed record keeping practices.

Maybe this will set a precedent and other states will start taking a harder
look at this issue, especially with the pending change to increase the use
of electronic medical records (EMR) and health records (EHR).

I think this points out what many have said previously though, simply moving
from physical format records to electronic records is not going to in and of
itself improve the quality of health care and diagnosis, it may in fact have
just the opposite effect if a physician lacks the attention to capture
information in the manner they were trained to do while being educated.
