Call Referral Logs (AG7-007): Documents all calls received by
   Community Access Unit and its contractors for use in process
   improvement. Records include call logs, referral logs, scheduling
   and IR tallies, and information and referral summary.
   Retention: Retain 5 years. (99-0025/010)

 Client Counseling Reports (AG7-006): Documents contracted service
   responses to calls from seniors or interested parties regarding
   services and/or care. The current contractor is Protocall. Reports
   show date and time of call, counselor's name, client's name and
   personal information, caller's name and relationship to client,
   identified problem, intervention response, and resolution. Also
   includes call logs.
   Retention: Retain 5 years. (99-0025/011)
