I agree that in principle having a different password for each site is the best approach. However, I currently have a file of electronic passwords for my various sites and have discovered that I have over 120 different sites that need a password. I couldn't possibly remember and keep up with updating that many different passwords. Many sites now require two-step authentication (a password and a separate identity-related question you have to answer). That would help, except that the second question is usually one I can't remember anyway (asking favorite color when I don't have one, asking about pet names from childhood when I had multiple pets, best friends from childhood when we moved a lot so I had more than one, etc.), lol. So the whole password protection part is really difficult anyway...
