There is no such thing as an industry standard or best practice percentage. The business, legal, and operational needs are so variable that it's not really possible to define one.

I am no statistician, but those who are have long advised that quality-controlling every image has no statistical validity, and serves no useful purpose (unless you need to grossly inflate the cost of your project).

Check out ANSI/AIIM TR34-1996 "Sampling Procedures for Inspection by Attributes of Images in Electronic Image Management (EIM) and Micrographics Systems" (http://webstore.ansi.org/RecordDetail.aspx?sku=ANSI/AIIM+TR34-1996). This Technical Report offers a menu of different sampling rates, from which you and your risk management team can choose the ones best suited to your needs.

If these are important records, be sure to use BOTH quality control procedures described in ANSI/AIIM MS 44-1988 (R1993) "Recommended Practice for Quality Control of Image Scanners" (http://www.techstreet.com/cgi-bin/detail?doc_no=aiim%7Cms44_1988_r1993_;product_id=1275).

Finally, if the retention period of these records exceeds 15 or so years, you should either retain the paper records, or output your newly-created electronic images to first-generation silver-gelatin microfilm for long term safekeeping.
