I have to side with PK on this.  I've reviewed a number final critiques of these types of projects. Some of the failures can be blamed on contractors for over promising results.  But much of it falls on the government agencies who write the contracts, in many cases develop the requirements (a component of planning) and oversee the project. They assign project managers who have little experience, underestimate the effort and resources required and request additional features or change the scope while the project is already underway.
