When I was at HP/Agilent, working in Process Engineering, we encountered a situation in which a component was routed in a loop through various engineering stations. After hunting around - and a lot of head-scratching - we got to the bottom of why there was this extra loop: it turned out that there had been a guy (10 years retired) working on that manufacturing line who was nearly ready to retire and who made mistakes. So, they designed in this loop, so somebody could check behind him. 10 years later, they were still following the SOP, put in place to correct for somebody who hadn't been there all that time.
