First, there is no reason to pay for a template Business Associate
Agreement (BA agreement) - many templates are available free of charge.


Second, note that the link below shows a form dated 2002 - which is
seriously out-of-date for a BA agreement.  The final Security Rule was
published in February 2003 and went into effect in April 2005.

One thing I'd DEFINITELY suggest you ensure you get is a "Business
Associate's Agreement", especially if you store anything covered by
HIPAA.
