that first provides a benchmark on how the people in the organization
(cross-sectional random sample) perceive Record Management behaviors,
both their own views and how they perceive the Company culture to be.
The intent is to then re-survey down the road to provide measurements

Easy to do - come up with no more then 20 questions that can be answered
either True False or multiple choice. 
Key points are to instruct people to answer them as they really see
things not as they think they should be (otherwise people tend to view
