Generally yes, an as-built should reflect what was actually in place.  Please note however, that the question may have multiple answers, depending on your point of view.  I believe the engineering firm that designed the St Louis Hyatt Regency walkways that collapsed in 1981 would argue that the as-approved documents are more valuable than the as-builts.  The same would be true for nearly every design firm.
