I am curious if any others on the listserv require vendors, upon contract termination, to complete a records destruction certificate to certify disposal?

Typically, in my experience, most vendor contracts contain some level of communication from the vendor validating disposal/transfer upon contract termination, but it really doesn't give the Records & Information Manager much in the way of really understanding whether or not information was properly disposed/transferred and what was actually disposed/transferred.

If you do, I would be interested in learning about your process and elements of your disposal form.
