It's good to know that forms management is still out there.  When I studied 
for the CRM Robek had an entire chapter devoted to Forms Management, and 
I thought as I read it, what happened to it?  No one I know does it with any 
level of diligence.  Perhaps it has followed the trail of other notable document 
types, such as email:

   - Well-managed in paper form
   - Emulated in electronic form
   - Hey - it's electronic - we don't have to manage it anymore!
   - Big, uncontrolled mess
   - Dollars and time spent to try to manage it

I'll have to look up the BFMA.
