When I was in the Army years ago, we would melt the hard drive in an incinerator, especially for computers used to store classified or confidential information.  A little radical for some, but the method was effective.

Typically, they are degaussed... especially if you have any desire to
re-use them.  In extreme situations, they are shredded...in industrial
shredders....seriously.
