Look at some of the public universities for examples.  My former
employer, Old Dominion University, has what I consider a solid policy
management program
(https://www.odu.edu/about/policiesandprocedures/university).  Policy
1001, Development, Approval and Maintenance of University Policy,
includes a template that you might want to consider.
