Carlo, the classic recent example is Pacific Gas & Electric (PG&E), which failed to keep up a successful pre-existing RIM program.
A related consequence was a gas pipeline explosion in San Bruno (CA) in 2010. Eight people were killed, many wounded, and property damage was consequential.
The company is still paying off the costs, both in money and loss of reputation.
The California Public Utilities Commission hired British RIM consultants to evaluate PG&E's RIM program. Their report is pretty damning.
