IMHO, every organization should have a written procedure for modifying its RRS. That procedure should include these steps:

Identify the needed change.
Validate the needed change.
Communicate the proposed change to stakeholders; obtain feedback.
Submit the change to the RIM/IG governing committee.
Implement the change: publish new RRS
Communicate the change to stakeholders: records users/owners, owners of applications coded with the record series, records storage vendors, third parties deemed to be records custodians.
Document the change in your RRS History file (or database)
