I've got one:

while departments work tirelessly to create and implement procedures to
properly identify, label and file electronic and paper records, the most
important company data gets 'purged' prematurely by the IT department to
backup tape - which is 50 year old technology -making it inaccessible when
it is needed most, which is in law suits and regulatory audits. That's like
putting the most valuable information in a big locked vault and throwing it
off a cliff. Meanwhile employee expense reports and requests for time off
are meticulously indexed and filed.
